Clinical Record Requirements

 

 

Documentation requirements vary among clinical agencies and students are expected to abide by the rules and regulations of the respective agency. All the following requirements must be met prior to participating in clinical and must remain current throughout the program of study. Information regarding how the requirements are obtained is provided upon admission to the program. 

  1. Background check
  2. Drug test
  3. HIPAA and Code of Conduct Training
  4. Infection control and universal precautions orientation
  5. BLS Healthcare Provider certification by American Heart Association (CPR)
  6. Physical examination
  7. Immunization records: TB skin test, hepatitis B series, MMR or rubella/rubeola titer, varicella,  Tdap and seasonal flu.
