Clinical Record Requirements

 

 Documentation requirements vary among clinical agencies and students are expected to abide by the rules and regulations of the respective agency. The required health and clinical record documentation must be submitted following notification of admission and maintained current through enrollment in the program. 

  1. Criminal background check
  2. Urine drug screen
  3. BLS Healthcare Provider certification by American Heart Association (CPR)
  4. Physical examination
  5. Immunizations, including TB skin test, hepatitis B, MMR, varicella, Tdap, and seasonal flu.
