How To Request an Acceptance Letter
Students can request an acceptance letter in person, or by faxing or mailing a completed request form.

Be sure to include or have the following information ready:
1. Full name and any name used while attending CWC.
2. Daytime phone number, in case there is a problem.
3. Social Security Number.
4. Signature.
5. Detailed addresses of where your acceptance letter should be sent, including the names of specific people to whose attention it should go.
